To make a sample request, log in to Merck Connect. Please ensure your Merck Connect profile contains NPI (National Provider Identifier) as the license type and provide your NPI number before visiting the sample request page to navigate to SampleCenter. If you are a first-time user of SampleCenter, you will be prompted to register on SampleCenter.

Available samples are shipped as soon as the appropriate authorization has been received. Eligible, validated MDs, DOs, and other licensed practitioners (depending on state-level requirements) can expect to receive samples within 3–5 business days of Merck receiving the signed and dated Sample Request Form.
